Ministry of Dance Evangelism (“MODE”) is committed to protecting our students. This Privacy Policy explains how we collect, use, and protect your information when you interact with our website or participate in our programs. MODE operates under the nonprofit covering of e2Ten, a 501(c)(3) organization.
This agreement is entered into between E2Ten (parent ministry of Ministry of Dance Evangelism and Kalos Agon) and the undersigned participant (or the parent/legal guardian of a minor participant).
1. Voluntary Participation
I understand that participation in dance, martial arts, fitness, or related activities at E2Ten (parent ministry of Ministry of Dance Evangelism and Kalos Agon) is voluntary and may involve physical exertion and risk of injury.
2. Assumption of Risk
I knowingly and freely assume all risks, both known and unknown, including but not limited to injury, illness, falls, collisions, equipment failure, or actions of other participants.
3. Release and Waiver
I release and discharge E2Ten (parent ministry of Ministry of Dance Evangelism and Kalos Agon), its owners, instructors, employees, and agents from any and all liability, claims, demands, or actions arising from participation, including claims of negligence, to the fullest extent permitted by law.
4. Medical Treatment
I authorize E2Ten (parent ministry of Ministry of Dance Evangelism and Kalos Agon) to obtain medical treatment in the event of an emergency and agree to be financially responsible for any related costs.
This release is entered into between E2Ten (parent ministry of Ministry of Dance Evangelism and Kalos Agon) and the undersigned participant (or the parent/legal guardian of a minor participant).
1. Consent to Photograph/Record
I grant permission to E2Ten (parent ministry of Ministry of Dance Evangelism and Kalos Agon) and its representatives to take photographs, video, or digital recordings of me (or my child) during classes, events, and performances.
2. Use of Media
I authorize the use of such media for promotional, educational, and marketing purposes, including but not limited to print publications, social media, websites, and advertisements, without compensation.
3. Waiver of Rights
I waive any right to inspect or approve the finished media or how it is used.
4. Duration
This release is perpetual, worldwide, and may only be revoked in writing.
